Rationale for Schedule

This schedule was created with the immunological development and individual health of the child, and lifestyle of the parents in mind.  It can be altered for individual cases as needed.  It does not follow CDC guidelines.  If the physician suspects that a parent is not breast feeding, is smoking, or has other risk factors, CDC guidelines may be more appropriate.

Hepatitis B

Hep B is not necessary at birth if the mother is Hep B surface antigen negative and Hep B core antigen negative.  All mothers must be tested during pregnancy.  Most newborns are not at risk for Hepatitis B unless mother is a carrier or has active Hep B.  Hep B is transmitted sexually, needle use, and blood.  It is not transmitted through breast milk.  Adolescents may be vaccinated before puberty if they were not vaccinated as infants.  All individuals in high risk groups for contracting Hep B should be vaccinated if they haven't been vaccinated before.  High risk groups include multiple sexual partners, men who have sex with men, IV drug users, healthcare workers, childcare workers, some immigrants etc.

Diphtheria, Tetanus, Pertussis (DTaP)

DTaP is recommended at 6 months for children in daycare of those who will be exposed to Diphtheria or Pertussis.  Vaccination at 12 months results in a much more robust immune response.  Titers can be done a 18and 24 months to determine if these shots are necessary.

Haemophilus Influenzae b (Hib) and Pneumococcal Conjugate (PCV)

Hib and PCV are recommended at 6 months of age or later for children who are breast fed, not in day care, and who are not exposed to second hand smoke.  Earlier vaccination is recommended for children who do not meet these criteria and for African American children.  Vaccinations before six months show no antibody titer.  Children who are breast fed do not need to be vaccinated for Hib or PCV until cessation of breast feeding.  Passive immunization is extremely effective for Hib. PCV may be recommended slightly earlier than Hib because exposure an disease tends to be earlier.

Polio (IPV)

Polio vaccination should not occur when the child is being breast fed.  Antibodies in breast milk will destroy the virus before the immune response occurs.  We recommend polio vaccine after 1 year of age and a booster shot at 4-6 years.  For all booster shots, parents can choose to measure titers to determine whether a booster is necessary.  Labs will give acceptable titer levels.

Measles, Mumps, Rubella (MMR)

We have recommended MMR at 24 months.  None of these diseases are severe in healthy infants. Furthermore, because of the low disease rate for these in the population, it will be rare for children to catch these diseases.  If contracted as adults, these diseases can be significant.  Therefore, vaccination is ultimately recommended.

Chicken pox (Varvax)

Chicken pox vaccine is only 70% effective.  Chicken pox is a disease easy to recover from.  Sequelae are rare.  As an adult, Chicken pox is a more serious disease.
